Physical Form

Throughout the year, opportunities include:

Girls: volleyball, basketball, flag football

Boys: flag football, basketball, volleyball

Requirements:

Maintain a GPA at/above 2.00.

Have submitted a current (within the last year) sports physical.  As the beginning of the year is busy for many medical offices, it is strongly encouraged to have this completed over the summer. A student may NOT try out, participate in practice, or play in competitions until he/she has submitted your physical exam to the coach and a copy has been filed in the School Office.

*Practices and games are typically right after school.  

*Transportation to games is arranged through parent drivers.

*While there is no required participation fee, the Athletic Department funds its programs (uniforms and supplies) primarily through parent donations.  To help with this, a $25 donation per sport is appreciated.

A student who is issued an administrator-assigned detention may be removed from one game.  A student becomes ineligible to participate in sports while under a temporary removal or suspension.  A student who has been temporarily removed or suspended from school and/or demonstrates unsafe or disruptive behavior may be removed from the team depending on administrative review. 

Coaches

Coaches interested in supporting Hillcrest Middle School’s after-school athletics program (including volunteer coaches) must have current First Aid and CPR certifications and be fingerprinted through the Gravenstein Union School District Office.  Coaches must also complete specific coaches’ training at their own expense.  For information on coaches’ training contact the athletic director, Matthew McDowell, at mmcdowell@gusdschools.org.  Parents or guardians who drive students, other than their own, to or from games must meet the Gravenstein Union School District’s requirements for insurance.  Forms are available in the school office.
